Please fill out the form below if you would like a personal name tag created for you to wear to Sunday services and other LOPC events. This nametag will be laminated with a clip.

Instructions

Please submit your request by end of day Thursday in order to receive your name tag by this Sunday. Requests made after Thursday will be fulfilled the following week on Sunday. Either way, you will receive an email notification once your name tag is available for pick up in the Welcome Center. Your name as it is spelled below is how it will appear on your name tag.
